Evaluator Forms

Below you will find two different evaluator forms based on the applicant's intended program. The email that you received from the applicant indicated which program he/she is applying to. Please choose the appropriate form below.

General Evaluator Form
This form can be used for the following programs:
Anesthesiologist assistant, biomedical/medical laboratory sciences, cardiovascular perfusion, journalism (all tracks), Master of Arts in Teaching (elementary & secondary), MBA (all tracks), molecular & cell biology, pathologists' assistant, nursing (all tracks), public relations, radiologist assistant, and social work

General Evaluator Form (writable PDF) - This form is a writable PDF. You will be able to electronically complete the form, save to your computer and follow the upload instructions to the unique URL for the applicant from the email that you received.

General Evaluator Form (standard PDF) - This form is a PDF that can be printed out, completed by hand and returned to us.

Educational Leadership Evaluator Form
This form should be used for an applicant applying to our Educational Leadership program.

Educational Leadership Evaluator Form (writable PDF) - This form is a writable PDF. You will be able to electronically complete the form, save to your computer and return to us via one of the methods outlined below.

Educational Leadership Evaluator Form (standard PDF)  - This form is a PDF that can be printed out, completed by hand and returned to us.



How to Return Forms
You can return the completed forms to the Office of Graduate Admissions in one of the following ways:
  • Scan and upload to the unique URL for the applicant from the email that you received*
    *For the Educational Leadership program, please return the form directly to Quinnipiac via email, fax or mail
  • Scan and email to graddocuments@quinnipiac.edu
